West Virginia Recruitment and Retention Community Project
Grants to eligible healthcare employers in rural West Virginia to provide loan repayment assistance to primary care health professionals who agree to practice in underserved areas.

Rural Health Care Program – Healthcare Connect Fund
Provides assistance to healthcare providers for eligible expenses related to broadband connectivity at a flat discounted rate of 65%. Participants can apply as a member of a consortium or a stand-alone entity.

Missouri Aim for Excellence Award
Award that recognizes hospitals in Missouri for their accomplishments, innovations, and outcomes for projects that address at least one of the three Triple Aim principles, which are population, quality, and cost/value.

Rural Healthcare Provider Transition Project (RHPTP)
Technical assistance and training for rural healthcare providers to build capacity in value-based care (VBC), with emphasis on efficiency, quality, patient experience, and safety. The program's goal is to guide small rural hospitals and Rural Health Clinics that are not currently participating in VBC to prepare for participation in a health system focused on value.

Sid W. Richardson Foundation Grants
Grants for nonprofit organizations in Texas that address education, health, human services, and arts/culture. Applications for health-related projects are accepted from throughout the state, including rural areas.

Indiana SIA Foundation Grants
Funding for capital projects related to health and welfare, arts and culture, and education in Indiana.

Arkansas Life 360 HOME Program
Arkansas Department of Human Services will partner with eligible Medicaid hospitals to implement intensive care coordination for Medicaid clients most at risk of poor health outcomes.

HUD Section 242: Hospital Mortgage Insurance Program
Loan and mortgage insurance program to help acute care hospital facilities with remodeling, expansion, modernization, equipment, refinancing, and acquisition. Small, rural Critical Access Hospitals are eligible to apply.

Louisiana Rural Health Scholars Program
A short-term rotation for healthcare professional students to gain experience in rural healthcare settings in Louisiana.
